At Hall & Prior we provide residential care to a number of residents who are funded by both Aged Care and the NDIS.

Whether you're looking for permanent accommodation or support while you transition to your permanent home we can support your clinical, therapeutic, personal care and community support needs.

From Hospital to Home (FH2H)

This program supports you to move from hospital to a permanent accommodation setting when you are ready for discharge. It helps you understand the support you need, plan the steps to get there and gives you a place to stay while doing it.

To be eligible you must be:

• Referred from a Perth metropolitan public hospital.

• Medically ready for discharge from hospital.

• Working towards the goal of moving into a permanent

accommodation option.

• Aged between 30-65 years.

• Eligible for funding through the National Disability

Insurance Scheme (NDIS) or other funding.

Short & Medium Term Accommodation

We currently have vacancies within our homes for STA or MTA stays for NDIS participants. These vacancies would suit participants that require access to 24-hour clinical and personal care. We can also support you during your stay with your NDIS Therapy needs via our Allied Health Team.

Our world-class Karingal Green Health & Aged Care Community in High Wycombe offers dedicated beds and services and provides age and lifestyle appropriate programmes and facilities for young people.

Karingal Green is a space for healing with access to state-of-the-art facilities including a wellness centre, hydrotherapy pool, gymnasium, café, cinema, allied and primary health clinic and private dining.

We also cater to specific dietary plans and have a variety of options to support your needs and preferences.
